The Cooperation Fund Foundation requests your price offer, taking into account the following requirements:
I. Description of the subject of the contract:
1) Provision of general consultancy (by a key general advisor) individually and/or in groups for participants of the Project “Incubation and Support of Social Economy in the Ostrołęka Subregion – IWES 3”, contract no. RPMA.09.03.00-14-b025/18-00, in the scope of, among others:
a) establishing a PS (taking into account various legal forms and types),
b) establishing SEEs (taking into account various legal forms and types),
c) registering PES activities,
d) external financing of PS and PES,
e) conducting statutory activities of PES,
f) economizing NGOs (taking up paid public benefit activities, economic activities),
g) support for the transformation of the enterprise into a PS,
h) developing an individual support path tailored to the client's needs,
i) monitoring the situation, supporting the development plan, organizing support in crisis situations,
k) assistance in identifying current needs and coordinating the support path.
The key general advisor is obligated to collaborate continuously with labor market institutions and social welfare and integration institutions throughout the individual support path. This collaboration involves, at a minimum, exchanging information with these institutions regarding the support provided to the client. They are also obligated to establish cooperation.

2) The purpose of the request for proposals is to select a Contractor, based on the Bidders’ offers, who will provide general consulting services in accordance with the provisions of the “Standards for Social Economy Support Centres”, constituting Annex No. 2.
3) The Contractor shall provide all necessary materials and equipment to provide the service, including a computer with Internet access and legal software necessary to conduct classes.
4) Number of hours of general consulting to be provided as part of the project: maximum 200 hours. The Ordering Party reserves the right to specify that, depending on actual demand, the final number of hours may be less than indicated above. The Contractor also undertakes to hold regular meetings with the project team. Meeting time with the project team is not included in the total
in the scope of hours allocated to consulting.
5) The contracting authority allows individuals (independently and personally performing the subject of the contract) and entities employing specialists to participate in the procedure – designating specific individuals to provide a given scope of specialist advice. Consortia are not permitted to submit contracts.
6) The Client does not incur any additional costs related to the provision of the service (e.g., accommodation, travel, meals). All costs related to the preparation, implementation, and settlement of the consulting service are borne by the Contractor.
7) The Contractor whose offer is selected as the most advantageous undertakes to enter into an agreement with the Ordering Party under the terms specified in Appendix 3 to this Request for Proposal. Based on the agreement referred to in the preceding sentence, the Contractor will be obligated to complete the order in accordance with its submitted offer.
II. Requirements for the Contractor (subjective criteria):
The contract may only be awarded to a Contractor who meets the following conditions (i.e. who has persons who meet the following conditions):
1. Higher education,
2. At least three years of professional experience in supporting the creation of PS or PES,
3. At least 300 hours of consultancy services carried out in the last 3 years before the deadline for submitting offers, for which the offer is submitted and whose recipients were people at risk of poverty and/or social exclusion and representatives of the Social Insurance Institutions,
4. At least 3 references from the last 3 years regarding the consultancy services provided by the Contractor on the indicated subject matter.

V. Place and date of order fulfillment:
Place
Ostrołęka subregion, i.e. the following counties: Ostrołęka, Ostrołęka, Ostrów, Przasnysz, Maków, Wyszków
The consultancy will be provided at the place where the need for consultancy arises, i.e. at the office of the Social Economy Support Centre and/or at a location convenient for the client (e.g. the seat of a social enterprise), or at another location indicated by the Ordering Party.
Advice will be provided primarily directly, only in exceptional cases and after prior arrangement with the OWES manager it is possible to provide advice indirectly (e.g. by email).
Deadline
Consulting services will be provided from the date the contract is signed until April 30, 2022. Consulting deadlines will be agreed upon in consultation with the selected Contractor. Consulting services will be provided on an ongoing basis, in accordance with the needs of OWES clients. Consulting support must be provided within three business days of the client's request for such services.
VI. Method of preparing the offer :
The offer should be submitted on the offer form constituting Annex 1 to this request for proposals.
The Offeror's offer should only include the price for 1 hour of consulting.
The offer must be completed on a computer and in Polish. Prices quoted in the offer must include all costs related to order fulfillment and cannot change during the contract term.
Bidders are required to carefully review the information contained in the request for proposals and its attachments and prepare their offer in accordance with the requirements specified in this document.
Offers prepared contrary to the requirements of this request for proposals, submitted on a different form, incomplete or after the deadline will not be evaluated.
Bidders bear all their own costs associated with preparing and submitting their bids, regardless of the outcome of the procedure.

X. The offer can be:

ul. Górnośląska 4a, 00-444 Warsaw – the date and time of receipt by the Foundation are decisive.
b. send by e-mail to the following address: jjakubiak[at]cofund.org.pl – the date of display of the message on the recipient's computer is decisive.
c. deliver it in person to the FFW Office at the following address: Fundacja Fundusz Współpracy, ul. Górnośląska 4a, 00-444 Warsaw – the date and time of receipt by the Foundation will be decisive.
XI. Information on planned supplementary orders
The Ordering Party allows for the possibility of supplementary orders, in an amount not exceeding 50% of the value of the public procurement specified in the contract.
XII. Other provisions
a. This request for proposals does not constitute an offer within the meaning of the Act of 23 April 1964 – Civil Code (Journal of Laws of 2017, item 459, as amended). The estimated value of the order does not exceed PLN 50,000.00 net.
b. Offer validity period: 60 days from the end of the offer submission deadline.
c. The Ordering Party reserves the right to withdraw from the execution of the Order that is the subject of the inquiry without giving any reason at any time, also after the submission and consideration of offers, and also reserves the right not to select the Contractor and not to sign a binding contract.
d. The Ordering Party may conduct price negotiations with the Contractor whose offer was selected.
e. The Ordering Party reserves the right to make changes to the content of this request for proposals and its attachments.
f. The Ordering Party reserves the right to contact (by phone, email, or in person) any Bidders whose offers require supplementation, clarification, or clarification within the timeframe set by the Ordering Party in accordance with the preceding sentence, to ensure comparability of offers. If a Bidder fails to provide supplementation, clarification, or clarification within the timeframe set by the Ordering Party, their offer will be subject to rejection.
g. After selecting the most advantageous offer, the Ordering Party will immediately call on the Contractor who submitted the most advantageous offer to conclude the contract. If the Contractor fails to conclude the contract within 7 days of being called to sign the contract, the Ordering Party may begin negotiations with the Contractor whose offer was next in line.
h. There is no possibility of appeal against the selection made.
XIII. Obligations of the Data Administrator related to the protection of personal data
1. The Controller of the personal data contained in the offer is the "Cooperation Fund" Foundation (hereinafter referred to as the "CONTROLLER"), with its registered office at ul. Górnośląska 4a, 00-444 Warsaw. The Controller can be contacted in writing, by traditional mail at ul. Górnośląska 4a, 00-444 Warsaw, or by e-mail at cofund@cofund.org.pl
2. The Administrator has appointed a Data Protection Officer who can be contacted at the following e-mail address: iodo@rt-net.pl
3. Personal data referred to in paragraph 1 are processed on the basis of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation), i.e. based on the consent of the data subject and the Act of 29 January 2004 – Public Procurement Law.
4. Personal data are processed for the purposes necessary to carry out the public procurement process.
5. Personal data are processed based on the legitimate interest of the controller or a third party.
6. Personal data does not come from third parties.
7. The Administrator does not intend to transfer data to a third country or international organization.
8. The Administrator will transfer personal data to other entities only on the basis of legal provisions, including in particular: the Public Procurement Office, the Social Insurance Institution, and the Tax Office.
9. Personal data will be processed by the Administrator no longer than necessary to achieve the purposes to which the Offeror consents.
10. The data subject has the right to request from the controller access to personal data, their rectification, erasure or restriction of processing, the right to object to the processing, as well as the right to transfer data.
11. A complaint about the Administrator's actions may be filed with the President of the Personal Data Protection Office.
12. Providing personal data is required to conduct the contractor selection process. Failure to provide it will result in the inability to submit a bid.
13. The Administrator does not envisage automated decision-making.
