The Cooperation Fund Foundation requests that you submit your offer, taking into account the following requirements:
Description of the subject of the order:
The subject of the order is a software programming and implementation service, which includes:
- design, implementation and after-sales service support of an integrated subpage – GENERATOR – an internal website of the project, requiring user registration and login, containing a mechanism for registering, entering, sending, editing and evaluating applications and cards (2 types of forms) – as described in Annex No. 1
CPV code(s) and name : 72000000-5 – IT services: consulting, software development, internet and support
Source of financing: Project "Cooperation Paths – Support for Entities Implementing International Cooperation", co-financed by the European Social Fund under the Knowledge, Education, and Development Operational Programme. Grant agreement number: POWR.04.03.00-00-0096/17-00.
Place and date of order execution: Warsaw, from signing the contract until August 30, 2020, creation of the platform no later than the end of March 2019.
Requirements for the Contractor (subjective criteria):
Possession of the necessary knowledge and experience as well as technical potential acquired through the implementation of the following services:
- 3 orders with a value of at least PLN 50,000 gross each, consisting in the provision of a service covering the design and creation of a database system, including at least one system supporting at least 1,000 users (the name of the system, system description, ordering party, number of users and at least 1 confirming reference are required);
- 1 order covering the design and creation of systems processing and storing large volumes of personal data, including the PESEL number – the name of the system, system description, ordering party, number of users and at least 1 confirming reference are required;
- at least 5 website orders in which innovative UX elements and aesthetics of implementation were an important element of the order,
- at least 5 graphic designs of websites, designed by the contractor,

Method of preparing the offer:
The offer should be submitted on the offer form constituting Annex 1 to this request for proposals.
The offer must be completed on a computer and in Polish. Prices quoted in the offer must include all costs related to order fulfillment and cannot change during the contract term.
Contractors are obliged to carefully read the information contained in the request for proposals and annexes and prepare their offer in accordance with the requirements specified in this document.
Offers prepared contrary to the requirements of this request for proposals, submitted on a different form, incomplete or after the deadline will not be evaluated.
Contractors bear all their own costs related to preparing and submitting their offer, regardless of the outcome of the procedure.
